Health authorities in the Democratic Republic of Congo (DRC) have reported that more than 2,000 people have died from a recent outbreak of the Bundibugyo Ebola virus strain. This outbreak, currently affecting the east and northeast regions, has also seen over 4,380 infections since its confirmation.

The 17th Ebola outbreak recorded in the country, it is notable for its rapid spread and high mortality rate, surpassing previous outbreaks, including the 2014–2016 West African epidemic. Local and international health officials warn the outbreak may become the largest ever recorded in the DRC.

The health crisis has been complicated by ongoing security issues, poor infrastructure, and healthcare strikes, all of which hinder efforts to track and contain the virus.

Outbreak Progress and Deadliness

The current outbreak was officially declared on May 15, though transmissions likely began earlier in the year. It took nine weeks to reach the first 1,000 deaths, but fatalities have since doubled in just three weeks. The case fatality rate stands at 45.9%, higher than the 39.6% fatality rate recorded in the 2014–2016 West African outbreak, which remains the deadliest on record globally.

Challenges in Containment and Vaccination

Authorities have imposed restrictions on public gatherings and closed the airport in Ituri Province, the epicenter of the outbreak. However, displacement caused by fighting and disease has spread populations within the region and into neighboring countries such as Rwanda and South Sudan.

Despite available vaccines against some Ebola strains, none are currently approved for use against the Bundibugyo strain. Clinical trials for antiviral treatments began in July, and the World Health Organization (WHO) has recommended a full-scale trial of the Ervebo vaccine, which is effective against the Zaire strain but untested against Bundibugyo. WHO hopes it might provide some protection.

Scientific Insights and Ongoing Risks

Scientific research suggests that the Bundibugyo strain likely originated from an animal-to-human transmission event before spreading between people. Given the outbreak’s severity, the possibility that the virus has mutated is under investigation.

Tracking contacts of infected individuals has become increasingly difficult, with fewer new cases linked to known transmissions. This complicates containment efforts and falls short of WHO targets for comprehensive contact tracing.

Official Perspectives and Outlook

The health minister of the DRC has stated that the outbreak peak has not yet been reached but anticipates that containment efforts will start to reduce the spread within three months. Meanwhile, the WHO regional director has described efforts as a race to catch up with a virus that remains ahead of authorities.
